What Youll Do

As a Home Care Occupational Therapist youll provide individualized evidence-based care in patients homes including:

• Evaluating patients functional abilities and developing personalized treatment plans
• Improving activities of daily living (ADLs) and promoting independence
• Conducting home safety assessments and recommending adaptive equipment or home modifications
• Providing therapeutic exercises and interventions to improve strength coordination and mobility
• Educating patients and caregivers on techniques to maximize safety and function
• Collaborating with physicians and interdisciplinary care teams
• Completing accurate timely electronic documentation
