Job Summary and Responsibilities

As our Home Care Supervisor you will help our home care professionals deliver high-quality patient care so they can ensure the health and well-being of our diverse clientele throughout the Sacramento Service area.

Every day you will supervise and motivate an interdisciplinary team acting as a clinical resource monitoring performance and ensuring compliance with all regulatory requirements. You will be expected to collaborate with a wide range of stakeholders to optimize patient care outcomes and achieve department-specific metrics.

To be successful in this role you must possess strong clinical decision-making skills excellent communication and interpersonal abilities and a comprehensive understanding of home health regulations and quality improvement strategies.

  • Collaborates with other area supervisors as required to support the hospitals role as a community health service provider.
  • Assists in implementing a unit Quality Management Program consistent with hospital wide Performance Improvement Plan. Operates the units within the financial and productivity guidelines setting goals and developing policies that support this objective.
  • Provides for quality services by ensuring that the staff are well trained in the services to be provided.
  • Addresses escalates or resolves unit issues including problem resolution related to customer complaints concerns and suggestions.

Job Requirements

  • Minimum of two (2) years of management/leadership experience (within the last 5 years) in a home health agency primary care clinic or health facility sufficient background knowledge and expertise in clinical decision making for the patient population assigned.
  • Active Registered Nurse (RN) license in the State of California
  • Basic Life Support (BLS) certification - American Heart Association
  • Associates Degree in Nursing

Preferred

  • Home Health Supervisory experience.
