Responsibilities:

• Provide a full scope of primary care including new admissions routine follow-up visits and comprehensive medical management for residents
  
• Collaborate with an interdisciplinary team to ensure high-quality patient-centered care
  

Requirements:

• Active Nurse Practitioner license in the state of NY
  
• Minimum of 6 months NP experience
  
• Prior nursing home or long-term care experience highly preferred
  

Schedule:

Full time Monday through Friday

Compensation & Benefits:

• Competitive annual salary ranging from $150000 to $155000
