Position Highlights

• Deliver comprehensive patient-centered care for a senior-only panel

• Monday to Friday schedule 8:00 AM to 5:00 PM with a dedicated lunch hour

• Patient volume: 10-15 per day allowing for quality unrushed visits

• Focus on preventive care chronic disease management and patient education

• Close collaboration with supervising physicians and a multidisciplinary care team

• Practice medicine in a model designed around outcomes not volume

Candidate Requirements

• Graduate of an accredited Nurse Practitioner program with active licensure

• Skilled in EMR documentation and committed to accuracy
