- Remote patient care: Conduct initial and follow-up telehealth assessments on flagged patients; initiate/adjust plans of care; coordinate with referring and primary care providersall via secure video/audio platform.
Data into action: Interpret trend-based insights from the Circadia platform prioritize high-risk patients and drive timely interventions to prevent decline and unnecessary ED visits.
Close the loop: Complete documentation/charts within the same day.
Tele-visit management: Efficiently complete 25 tele visits per day for designated home health patients in the SoCal/OC area focusing on new admissions high-acuity flagged patients and Circadia alerts.
Quality & product feedback: Surface insights to improve virtual workflows and tech integration in a home health setting.
- Mentor & collaborate: Support NP onboarding share best practices and partner with clinical ops/product teams.
Visits: Target 25 patient tele visits/day
Flagged Patients are addressed within 24 hours
Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
Adoption & impact: Demonstrated workflow adherence across patient panels; trend toward fewer falls/hospitalizations and improved home health star ratings over time.
- Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation; company bills on your behalf.
- 2 years NP experience with complex adult populations in home health or post-acute settings; able to practice independently.
- Comfortable conducting telehealth visits and navigating virtual care platforms.
- Thrives in a dynamic evolving startup environment.
- English required.
- Tech-savvy: Google Suite multiple EMRs (HCHB experience a plus) Slack AI transcription/tools.
- Spanish preferred.
As a full-time Nurse Practitioner you will be employed by Circadia Health Inc. The anticipated annual base salary range for this full-time position is$140000 - $180000. The base range is determined by role and level and placement within the range will depend on a number of job-related factors including but not limited to your skills qualifications experience and location.
