Chi-Care is a nonprofit mobile medical clinic bringing compassionate accessible care to individuals experiencing homelessness in Chicago. We believe in meeting people with dignity respect and trust.
Position Overview
The NP or PA delivers frontline clinical care during mobile outreach shifts focusing on acute urgent care infectious disease screening vaccinations and harm reduction education. Working alongside clinical providers Peacekeepers and outreach staff the NP/PA serves as a trusted bridge to medical and social services across van-based and street medicine sites throughout Chicago.
Responsibilities
  • Conduct physical assessments and provide appropriate treatment for acute and chronic conditions during street outreach and mobile van shifts
  • Administer vaccinations dispense medications from the Chi-Care Med formulary and provide prescriptions within scope of services
  • Perform infectious disease point-of-care screening and wound care as needed
  • Ensure accurate timely documentation in the electronic health record (EHR) and maintain compliance with confidentiality standards
  • Identify patients social needs and collaborate with social workers to connect patients with partner organizations and higher-level care
  • Participate in team meetings trainings and quality improvement efforts; support students and volunteers as needed
Qualifications
  • NP: MSN and current Illinois Advanced Practice Nurse (APN) License with full practice authority;
    PA: MPAS (or equivalent) and current Illinois Physician Assistant License
  • BLS/ACLS certification (required)
  • Current drivers license with reliable transportation (required)
  • Minimum 5 years of clinical experience; community health emergency or primary care preferred
  • Knowledge of harm reduction principles and comfort working in non-traditional street-based settings
  • Experience with people experiencing homelessness substance use or mental health conditions strongly preferred
  • Excellent interpersonal skills; empathetic and aligned with Chi-Care Meds mission of meeting people with dignity
  • Spanish language skills preferred but not required
