As a Nurse Practitioner with CommonSpirit Health at Home youll provide face-to-face assessment services to patients to assist the Medical Director and team to determine each patients continuing eligibility for palliative care.
Responsibilities:
- Provides care following the plan of treatment and patient care plan to assist in determination of palliative care eligibility.
- Establishes a plan of care for continued needs based on the client assessment.
- Reports changes in patient condition to appropriate personnel and the attending physician.
- Provides patient care education counseling medication advice instructions and follow-up to clients and other caregivers.
- Participates in coordination of services with other disciplines and community resources as needed.
- Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment.
- Assists with the development of educational programs in the palliative care program for diverse age groups.
- All other duties as assigned.

- Must have full prescriptive authority
- Graduate of an accredited school of nursing.
- Currently licensed to practice in the state serviced with Masters in nursing.
- At least 1 year of nursing experience
- Must obtain and maintain current CPR
- Strong critical thinking clinical expertise case management and a passion for patient care.
- Valid drivers license and auto insurance coverage according to the CommonSpirit Health at Home policy.
