What Youll Do
As a Nurse Practitioner or Physician Assistant you will care for medically complex older adults in outpatient clinics and their homes. You will work as an embedded member of an interdisciplinary care team to manage chronic conditions support transitions from the hospital address psychosocial needs and prevent avoidable acute-care admissions.
You will conduct Medicare annual wellness visits review medical histories and medications perform screenings develop care plans and educate patients and families. You will also complete HCC coding and risk-adjustment documentation coordinate follow-up care and maintain timely accurate medical records.
The role typically involves 1012 clinic visits or 56 home visits per day. Home visits take place within approximately 30 miles of the Salem region. You must complete visit documentation within 72 hours.
Schedule Highlights
- Full-time MondayFriday 8:00 a.m.5:00 p.m.
- Current Oregon Nurse Practitioner license and ANCC or AANP certification
- Experience with high-risk patients acute care home health or underserved populations is preferred.
- At least two years of Nurse Practitioner experience in primary care family medicine internal medicine gerontology palliative care or a related specialty
- Knowledge of chronic-care management medication options Medicare HCC coding and managed care
- Active CPR/BLS certification and a current Oregon drivers license
- Ability to obtain DEA registration and prescriptive authority after hire
- Ability to travel throughout the Salem area five days per week
