Nurse Practitioner or Physician Assistant (Memory Care)

Job Summary
We are seeking a Physician Assistant or Nurse Practitioner to join the M Physicians Memory Care team. This is an outpatient role and will be practiced at the M Physicians St. Louis Park location.
Practice Details:
- Conduct comprehensive cognitive assessments including history-taking neurological exams and validated screening tools.
- Develop and adjust individualized care plans in collaboration with your Memory Care neurologists and others on the treatment team.
- Provide education and counseling to patients and caregivers on disease progression treatment options and community resources.
- Respond to urgent or emergent neurological issues related to prescribed treatments such as anti-amyloid therapies to evaluate and triage next steps.
- Participate in interdisciplinary case conferences and contribute to quality improvement initiatives in memory care.
- Ongoing collaboration with all members of our care team patients and families
Additional Practice Details:
- Full time 40 hours per week
- Monday Friday
- No current weekend or holiday requirements

The full-time salary range is $125759 - $155660 commensurate with practice experience and may also include incentive compensation or additional specialty-specific compensation components.

Position Qualifications:
A qualified candidate will be a graduate of an accredited school of nursing and formal Nurse Practitioner training program or a graduate of an accredited Physician Assistant program. Have a current MN RN license and be approved or eligible by the Minnesota Board of Nursing for prescription writing privileges or a current PA license with prescriptive authority through the MN Board of Medical Practice. Be certified through ANA / AANC or eligible for ANA/AANC national exam or through the NCCPA and meet threshold criteria/qualifications for credentialing and privileges.
Preferred Qualifications:
3 years as an advanced practice provider experience in a neurologic care setting is preferred. New graduates with an interest in neurology are encouraged to apply.
