Position Overview:As a Palliative/Hospice Nurse Practitioner atSalus Hospice you will work closely with aphysician and interdisciplinary teamto providecompassionate patient-centered care. Youll help ensure hospice patients meet theface-to-face visitrequirements and will support the team in delivering exceptional care to patients and their families.

Key Responsibilities:

  • Conductface-to-face patient visitsto evaluate continued hospice eligibility in line with new regulations.

  • Collaborate with physiciansand the interdisciplinary team to share clinical findings and adjust care plans.

  • Performbedside evaluations creating comprehensivecare planstailored to the patients needs.

  • Provide directhospice care supporting both the patient and family through sensitive and crucial moments.

  • Document all findingsper CMS guidelines ensuring accurate compliance.

  • Consult with supervising physiciansand collaborate with healthcare providers within and outside of Salus Hospice.

What Were Looking For:

  • 2 years of recent experienceinhospice or palliative care

  • Expertise in providing care topatients with end-stage disease.

  • Masters degree in Nursingpreferred.

  • Active Nurse Practitioner certificationin California.

  • California state licensureto practice as a Nurse Practitioner.

  • Willingness to travelwithin a large geographic area to visit patients.

Qualifications:

  • Experience in delivering high-quality compassionatehospice care.

  • Deep understanding ofend-of-life careand regulations.

  • Ability to followStandardized Proceduresbased on guidelines established by theBoard of Registered Nursing Practice Act.
