Job Summary

Shift: Weekday schedule only - no evenings weekends or holidays required; Flexible
scheduling options available including either four 10-hour shifts or five 8-hour shifts
based on operational needs
Requirements: Current Nurse Practitioner license in the State of Ohio; Family Nurse
Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) certification; ANCC
certification; Minimum of 1 year of Nurse Practitioner experience in a primary care or
hospital medicine setting; Current BLS/CPR certification; DEA certification or ability to
obtain; Board certification.
