Job Summary

Diagnostic Group is seeking a full-time Nurse Practitioner to join our medical team in Beaumont Texas.

This position provides comprehensive care to patients in the nursing homes with acute and chronic medical conditions as well as preventive and routine healthcare needs. While the primary focus of this position is nursing home care inpatient coverage is also required as needed including participation in weekend and holiday rotations and coverage for other inpatient providers when necessary. The ideal candidate will be highly organized comfortable working independently and possess strong clinical assessment communication and critical-thinking skills with the flexibility to provide high-quality care in both outpatient and inpatient settings.

Essential Duties and Responsibilities:
Nursing Home Visit Responsibilities
  • Conduct comprehensive patient assessments medical histories and physical examinations in the nursing home.
  • Diagnose and manage acute and chronic medical conditions.
  • Develop individualized treatment and ongoing care plans.
  • Order and interpret laboratory studies imaging and other diagnostic testing as clinically indicated.
  • Prescribe medications or therapeutic treatments when appropriate and in accordance with Texas scope-of-practice requirements.
  • Perform medication review and reconciliation and identify potential medication-related concerns.
  • Provide preventive health services routine screenings and health maintenance.
  • Evaluate patients following hospitalization and identify concerns that may increase the risk of complications or readmission.
  • Provide patient and family education related to the patients current condition medications treatment recommendations and follow-up needs.
  • Identify changes in patient condition and determine when additional evaluation or a higher level of care is appropriate.
  • Monitor patient progress and adjust treatment plans as clinically appropriate.
  • Coordinate referrals or follow-up services when clinically indicated.
  • Collaborate with physicians and other members of the healthcare team to ensure continuity and coordination of care.
  • Maintain timely accurate and complete documentation in the electronic health record (EHR).
Outpatient Clinic Coverage
When assigned to outpatient clinic coverage the Nurse Practitioner will:
  • Evaluate and treat patients with acute and chronic medical conditions.
  • Provide routine and preventive care within the scope of the assigned clinic.
  • Order appropriate diagnostic testing and provide treatment based on clinical findings.
  • Coordinate care with physicians specialists and other healthcare providers as appropriate.
Inpatient Responsibilities
When assigned to inpatient coverage the Nurse Practitioner will:
  • Assist Hospitalists with new admissions hospital rounds and discharges.
  • Evaluate and assist in the management of acute and chronic medical conditions in hospitalized patients.
  • Review laboratory results imaging medications and other diagnostic information to assist with patient management.
  • Monitor patient progress and promptly communicate significant changes in condition to the Hospitalist.
  • Collaborate with physicians nursing staff case management specialists and other members of the multidisciplinary healthcare team.
  • Assist with discharge planning medication reconciliation patient education and coordination of follow-up care.

Qualifications:
Licensure & Certification
  • Current and unrestricted Advanced Practice Registered Nurse (APRN) license or equivalent advanced practice license in the State of Texas.
  • Current applicable national board certification as a Nurse Practitioner.
  • Current Basic Life Support (BLS) certification required.
  • Advanced Cardiovascular Life Support (ACLS) certification preferred.
  • Valid drivers license and reliable transportation required for patient nursing home visits.
Clinical Skills
  • Strong physical assessment clinical decision-making and critical-thinking skills.
  • Ability to independently perform comprehensive and focused patient evaluations in a facility setting.
  • Ability to identify and address specific clinical needs while maintaining continuity with the patients established healthcare providers.
  • Strong medication review and reconciliation skills.
  • Ability to recognize changes in patient condition and appropriately escalate care when necessary.
  • Strong organizational time-management communication and documentation skills.
  • Proficiency with electronic health records (EHR).
  • Ability to work effectively across nursing home-based outpatient and inpatient clinical settings.
  • Previous primary care family medicine internal medicine home-based medical care or inpatient experience preferred.

Schedule:
  • Full-time position
  • Primary schedule: 8:00am - 5:00pm; Monday - Friday
  • Primary responsibilities consist of nursing home patient visits.
  • Participation in weekend and holiday inpatient rotations is required.
  • Additional outpatient / inpatient coverage may be required as needed to support staffing needs including coverage for scheduled absences or PTO.
