Hoag Health the top-ranked health system in Orange County CA is seeking a Nurse Practitioner to join our growing Memory and Cognitive Disorders (MCD) program in Newport Beach CA. This role comes at an exciting moment of growth: the MCD program is moving into a newly inaugurated Center for Brain Health launched with a transformative $50 million donation. The Center will be a destination for comprehensive dementia care collaboration and innovation.


Role Details

  • Schedule: Full-time MondayFriday
  • Setting: Outpatient-based position
  • Location: Newport Beach CA and Irvine CA
  • Conditions: Memory & Cognitive disorders (Dementia Alzheimers Lewy Body Frontotemporal Vascular)
  • Team: Cognitive & Memory Physician Nurse Practitioner Administrative support staff
  • EMR: Epic

Key Responsibilities

  • Provide outpatient specialty care for patients with dementia and related neurodegenerative conditions including Alzheimers disease frontotemporal dementia (FTD) Lewy body dementia mild cognitive impairment and other complex cognitive presentations
  • Collaborate closely with Cognitive & Memory Physician and other specialists to deliver individualized evaluation and treatment plans
  • Support patients and families with clear communication counseling and longitudinal care planning

Requirements:

  • Nurse Practitioners or Physician Assistants considered
  • 3-5 years of experience as a Nurse Practitioner or Physician Assistant managing Neurology or Geriatric patients
  • Demonstrated experience managing dementia and neurodegenerative disease
  • Graduate of an accredited Nurse Practitioner training program
  • California Nurse Practitioner License (active and in good standing)
  • Strong communication and interpersonal skills and a commitment to compassionate family-centered care
  • Ability to thrive in a multidisciplinary team-based environment
