Location: Somerset New Jersey

A growing palliative care organization is seeking an experienced Nurse Practitioner to provide high-quality patient care while helping lead and mentor a team of advanced practice providers across New Jersey.

This hybrid role combines direct patient care with clinical leadership and offers the opportunity to make a meaningful impact on patients living with serious illnesses and complex chronic conditions.

Provide palliative care assessments and follow-up visits
Manage patients with serious illness and complex chronic conditions
Facilitate goals-of-care and advance care planning discussions
Collaborate with physicians specialists hospitals and community providers
Mentor and support Nurse Practitioners and clinical staff
Participate in quality improvement and operational initiatives
Assist with onboarding training and team development

Active New Jersey Nurse Practitioner license
National board certification and DEA registration
Minimum 5 years of clinical experience in palliative care hospice geriatrics oncology family medicine internal medicine or chronic disease management
Strong communication and leadership skills
Experience working within interdisciplinary care teams

Leadership or supervisory experience
Hospice & Palliative Care certification (ACHPN)
Experience with home-based care telehealth or value-based care models
Bilingual skills are a plus
