Grafton Integrated Health Network is seeking a compassionate and collaborative Nurse Practitioner to join our team at our Cold Spring Minnesota PRT. Enjoy a small patient caseload of approximately 18 clients a flexible hybrid work schedule and the opportunity to work in tandem with an experienced psychiatrist to provide person-centered care for children with complex behavioral health developmental and intellectual disabilities.

As a Nurse Practitioner you will provide comprehensive person-centered medical care to individuals with behavioral health intellectual and developmental disabilities. Provides general medical care and treatment to Graftons client population (children and adolescents) under supervising authority of a physician. Assesses the physical/emotional needs of individuals with developmental disabilities and psychiatric conditions. Prescribes or recommends drugs or other forms of treatment. Supports provision of a therapeutic milieu by working in collaboration with academic and residential administrators. Provides direct services as defined in the scope of practice that pertains to his/her license. Spends 80% of work hours providing direct billable services such as physical examinations same-day sick client visits etc. Reports to Executive Director administratively. Practices under supervising physician.

ESSENTIAL JOB FUNCTIONS

• Conducts admission file reviews for the organization
• Provides admission and/or annual physicals
• Provides same-day visits for sick clients
• Identifiesneed for initiates and coordinates referrals for specialty consultations; Coordinates follow-up with external medical specialists
• Medication Evaluation and Review Assess clients todetermineif pharmacotherapy is medicationfor client condition conduct periodic medication review tomonitormedication effectiveness andpossible sideeffects adjust medication regime according to client needs
• Orders and interpretsappropriate laboratorystudies x-rays electrocardiograms and other special examinations/testing toidentifyand assess clients clinical problems and health care needs
• Discuss clients with supervising physician as scheduled
• Attendmulti-disciplinary team meetings as needed to consult withthe treatmentteam about the course of with regional team in providing leadership on treatment issues
• Documents client files records physical findings and formulates plan of care based on clients condition
• Signs verbal orders within 24 hours of giving the order; communicates any concerns about the procedure with the assigned treatment team
• Counsels andadvisesclients and families about physical developmental and psychosocial aspects of health through individual and group contacts to promoteoptimalhealth
• Contributes to the health education of individuals and groups and applies methods designed to increase each persons motivation to assume responsibility for his/her own healthcare.
• Model a positive/professional leadership attitude when dealing with others in a day-to-day work situation
• Participates as an active member of the Medical Committee
• Performs other duties as assigned.

MINIMUM QUALIFICATIONS

• Requires aMasters degree in nursing.
• Minimum twoyears experiencein the field.
• Must hold a valid Minnesota nursing license as issued by the Boards of Nursing and Medicine.
• Must posses Advanced Practice Nursing (APRN) license to practice autonomously.
• Must hold a Professional Certification in both Pediatric or Family AND Psychiatric by examination consistent with the area of specialty (consideration may be given to individuals who hold a certification in one specialty with the ability to obtain the second upon hire).
• Employeemustpossessa valid drivers license.
