Nurse Practitioner 4 Day Work Week

We are seeking a Part-Time Nurse Practitioner (NP) to provide inpatient medical care in a Skilled Nursing Facility. This is an excellent opportunity for a provider who enjoys building relationships with patients collaborating with interdisciplinary teams and maintaining a flexible work-life balance.

• Part-time schedule 4 days per week
  
• Daytime hours with no evening clinic responsibilities
  
• Manage and round on patients in a Skilled Nursing Facility

• Perform daily inpatient medical rounds in a Skilled Nursing Facility.
  
• Evaluate diagnose and manage acute and chronic medical conditions.
  
• Develop and implement individualized treatment plans.
  
• Coordinate care with physicians nursing staff therapists specialists and family members.
  
• Complete timely and accurate documentation within the EMR.
  
• Participate in quality improvement initiatives and ensure compliance with facility and regulatory standards.
  
• Provide compassionate patient-focused care that promotes positive clinical outcomes and reduced hospital readmissions.
  

• Current Nurse Practitioner license in the state of practice.
  
• National Board Certification (AANP or ANCC).
  
• DEA license (or ability to obtain).
  
• Experience in Skilled Nursing Geriatrics Internal Medicine Family Medicine Hospital Medicine or Post-Acute Care preferred.
  
• Excellent communication and clinical decision-making skills.
  
• Ability to work independently while collaborating with an interdisciplinary care team.
