How youll contribute
As a Nurse Navigator (RN) you will serve as a patients guide through the healthcare system helping coordinate care remove barriers and improve the overall patient experience.
Job Duties
- Serve as the primary point of contact for patients and families
- Coordinate appointments testing treatments and referrals
- Provide education about diagnoses treatment options and available resources
- Assess barriers to care and connect patients with community resources and support services
- Advocate for patients and helping ensure timely appropriate care across the continuum
- Provide hands-on patient care including but not limited to vitals patient intake in-clinic procedures
- Cover other clinics as necessary
- Other duties as assigned

Qualifications and requirements
- Strong clinical judgment and critical thinking.
- Ability to work independently in a fast-paced setting.
- Calm confident decision-making under pressure.
- Clear communication with patients and care teams.
Qualifications
- Standard ADN and BSN preferred
- Outpatient Clinic preferred
- ACLS and BCLS certifications required.
