How youll contribute
As a Nurse Navigator (RN) you will serve as a patients guide through the healthcare system helping coordinate care remove barriers and improve the overall patient experience.

Job Duties

• Serve as the primary point of contact for patients and families
• Coordinate appointments testing treatments and referrals
• Provide education about diagnoses treatment options and available resources
• Assess barriers to care and connect patients with community resources and support services
• Advocate for patients and helping ensure timely appropriate care across the continuum
• Provide hands-on patient care including but not limited to vitals patient intake in-clinic procedures
• Cover other clinics as necessary
• Other duties as assigned

Qualifications and requirements

• Strong clinical judgment and critical thinking.
• Ability to work independently in a fast-paced setting.
• Calm confident decision-making under pressure.
• Clear communication with patients and care teams.

Qualifications

• Standard ADN and BSN preferred
• Outpatient Clinic preferred
• ACLS and BCLS certifications required.
