Join our Cardiothoracic Surgery team as a Nurse Navigator RN and make a meaningful impact on patients throughout their surgical journey! This primarily outpatient role is fastpaced phonefocused and centered around care coordination patient education and supporting highquality outcomes.

Youll serve as a trusted guide for patients and familiescoordinating appointments providing pre and postop education triaging calls and ensuring every patient feels supported and informed. Youll also collaborate with surgeons APPs and inpatient teams to keep care plans moving smoothly.

Schedule:Full-Time 40 hours/week Monday - Friday 7:30am - 4:00pm

Key Responsibilities:

  • Provides comprehensive pre and postoperative patient education drives quality efforts to reduce readmissions and manages procedure scheduling.

  • Implement structured postprocedure tracking to reduce readmissions.

  • Support daily CV clinic operations within budget and staffing guidelines.

  • Prepare the department for increased patient volume including the arrival of a new CV surgeon.

  • Strengthen quality outcomes patient access and provider support.

  • Provide CV casescheduling support as procedural volume grows.

  • Offer crosscoverage within the CV clinic to maintain patient flow and operational efficiency.

  • Offload nonprocedural and carecoordination tasks from APPs to enhance access and allow topofscope practice.

Qualifications

  • Licensure: Current RN (Registered Nurse) license in state of Missouri.

  • Required Education:Graduate from an accredited school of nursing

  • Preferred Education: Bachelor of Science in Nursing (BSN)

  • Required Certifications:Active BLS (Basic Life Support) certification

  • Preferred Certifications: ACLS Advanced Cardiac Life Support
