Job Summary:

The Nurse DocumentsCoordinatorfunctions asaDocuments Team staff member and the interdisciplinary clinical patient care team within an infusion pharmacy office for managing nursing documentation received from our contracted agencies and field nurses in collaboration with infusion office pharmacists Intake team members Nursing Case Managers Nursing Coordinators and Accounts Payable Department.

Essential Job Responsibilities:

  • Oversees home infusion nursing documentation from nursing visits provided to patients with acute and chronic diseases.
  • Knowledge of IV access devices patient assessments multidisciplinary care planning patient education IV medication administration laboratory values and ongoing clinical patient monitoring.
  • Fosters new and existing relationships and maintains contact with other industry healthcare professionals for educational purposes and potential partnerships.
  • Reports to Nursing Case Managers to identify patient and caregiver home infusion care appropriateness patient response to care and helps to develop nursing care plans in partnership with prescribers RN field staff and other members of the patients healthcare team to ensure optimal patient outcomes.
  • Remains current on new therapies industry trends and accreditation standards for home infusion services.

Position Qualifications:

  • Currentpracticalnurse license in good standing.
  • A minimum of 3 years nursingexperience is required.
  • 1-2years experiencein a home care setting preferred.
  • Working knowledge of insurance industry requirements preferred.
  • Practical knowledge of State and Federal regulatory requirements and knowledge of ACHC regulations as it applies to infusion home care is essential.
  • Must have strong organizational skills and liaison capabilities with well-developed written and verbal communication skills.
  • Able to work independently with minimal supervision.

Physical Demands:

Required to stand walk sittalkand hear; required to use hands to operate vehicles and office equipment;must be able to occasionally lift and/or move up to 25lbs; specific vision requirements for this job include: close distance and peripheral vision and the ability to adjust focus reasonable accommodationsmay be made to enable individuals with disabilities to perform the essential functions of this position without compromising patient care.
