Job Summary
Provide case management and residential rehabilitation services to adults with serious mental illness living in supportive housing. This role delivers individualized recovery-oriented clinical support in the community helping tenants maintain housing stability develop independent living skills manage medications and finances and achieve personal recovery goals. Responsibilities include conducting assessments developing and monitoring service plans coordinating care with treatment teams and natural supports providing crisis intervention performing home visits maintaining clinical documentation and supporting tenants in meeting the responsibilities of tenancy.
