Job Summary
- Serve as Qualified Mental Health Staff (QMHS) in compliance with NYS OMH regulations
- Manage a caseload and develop Individualized Action Plans (IAPs)
- Provide daily living skills training (ADLs) coaching and recovery-based support
- Monitor and document residents mental emotional and behavioral health
- Provide crisis intervention and trauma-informed support
- Oversee medication administration documentation and med counts
- Coordinate care with healthcare providers social services and community resources
- Facilitate groups and skill-building activities
- Maintain a safe structured home-like environment
- Complete accurate documentation and Medicaid billing records
- Bachelors degree in Human Services or related field
OR - Associates degree 3 years experience in a mental health setting
- Experience working with individuals with serious mental illness (SMI); co-occurring SUD/DD preferred
- Strong communication and documentation skills
- Ability to remain calm and make sound decisions in crisis situations
- Team-oriented with the ability to work independently
- Compassionate respectful and non-judgmental approach
- Valid NYS Drivers License with acceptable driving record
- Reliable transportation for client-related travel
- Ability to navigate stairs and respond to emergency situations
