Mental Health Professional (MHP) PM Shift

What you will do in this role: Mental Health Professional (MHP) provides group and individual psychiatric rehabilitative and case management services to adults with a history of multiple psychiatric hospitalizations and in need of long-term care stabilization.  You will participate with the interdisciplinary teams in developing and implementing therapeutic services and interventions.

Qualifications :

Mental Health Professional (MHP): Other centers may refer to this role as a Psychosocial Rehabilitation Services Coordinator Psychiatric Rehab Coordinator Psychiatric Rehab Counselor or Mental or Behavioral Health Counselor

What we need from you:

• Prior experience as a PRSC or mental health professional in a licensed nursing home intermediate care facility or specialized mental health rehab facility is preferred
• Bachelors Degree in a human services field (including but not limited to sociology special education rehabilitation counseling or psychology) and have a minimum of 1 year of supervised experience in mental health or human services is preferred.
• Minimum qualifications are an unrelated Bachelors degree and a minimum of 2 years in a mental health profession/role or a high school diploma and 5 or more years in the mental health profession with clinical supervision. 
• Experience with Point Click Care (PCC) is ideal but not required

This center is managed by TRANSITIONAL CARE MANAGEMENT.  We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs) Short-term Care Rehabilitation Intermediate Care (ICFs) and Behavioral Health Service (SMHRFs) centers.
