Job Description:

A safe recovery at home often depends on more than medical care. Patients may also need emotional support financial resources transportation housing assistance or help navigating community programs.

Elara Caring is hiring a PRN Medical Social Worker to support home health patients and families throughout Portland and surrounding southern Maine communities.

You will work independently in the community while receiving support from an experienced interdisciplinary team. Every visit gives you an opportunity to remove barriers advocate for your patients and help families move forward with confidence.

• Assess patients psychosocial financial and environmental needs
• Complete initial social work evaluations and follow-up visits
• Provide counseling education and emotional support to patients and caregivers
• Connect patients and families with community programs and financial resources
• Identify barriers that may affect a patients safety or ability to remain at home
• Participate in developing and updating individualized Plans of Care
• Collaborate with physicians nurses therapists and other care team members
• Support discharge planning and continuity of care
• Communicate changes or concerns affecting patient safety and well-being
• Complete accurate and timely clinical documentation

• Masters or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education
• Current Maine Social Worker license in good standing
• At least one year of social work experience in a healthcare setting
• Strong communication counseling documentation and advocacy skills
• Ability to work independently in patients homes
• Willingness to travel throughout the assigned service area with 50% travel required
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations

The base salary for this position is$60.00 to $70.00 per point based on the companys good faith estimate at the time of posting. Actual pay will be determined based on factors such as education experience skills and internal equity.
