Overview:

Sentara RMH Medical Center is now hiring a Medical Social Worker MSW. This is a position that will be responsible for the provision of medical social work services to patients receiving care in the hospital.

Education

Masters Degree Social Work (Required)

Certification/Licensure

• Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI within 90 days of hire (Required)
• Certified Case Manager (CCM) - Certification - Commission For Case Manager Certification (CCMC) within 1 year of hire (Required)

Experience

• 1 year of case management experience (Required)
• 1 year of acute care experience (Required)
• Experience with discharge planning (Preferred)

Duties/Responsibilities

• Collaborates with patients families healthcare team members and community organizations and using strong advocacy skills identifies needs and resources in the hospital and community.
• Develop and evaluates effective transition plans taking into consideration the client diagnosis prognosis course of treatment past and present services short-term and long-term goals provider options and/or available health care benefits as appropriate to the continuum of care.

We provide market-competitive compensation packages inclusive of base pay incentives and benefits. The base pay range for full-time employment is $72737.60 - $110323.20 annually. Additional compensation may be available for this role such as shift differentials standby/on-call pay overtime shift premiums extra-shift incentives or bonus opportunities. Compensation within the range may vary based on qualifications experience location market conditions and business needs. Note: If an annual salary is posted it is based on a full-time colleague working 2080 hours annually.
