Requirements for Medical Director Elderly Care

  • Completion of a physician residency program accredited by the Accreditation Council for Graduate Medical Education (ACGME).
  • Masters degree or doctoral degree as applicable to the physicians professional credentials.
  • Board certification in Internal Medicine Family Medicine or Emergency Medicine.
  • Geriatric medicine experience training or certification is strongly preferred.
  • Active unrestricted physician license in the applicable state.
  • Current state Controlled Dangerous Substances (CDS) certification as applicable.
  • Current DEA registration
  • Eligible for or currently enrolled as a provider with Medicare and Medi-Cal
  • Demonstrated knowledge of chronic disease management and geriatric medicine preferred.
  • Ability to work collaboratively within an interdisciplinary care model.
  • Willingness and ability to participate in a rotating night and weekend on-call schedule.
  • Ability to provide care in multiple settings including the PACE center participant homes hospitals and nursing facilities.

Unlike traditional fee-for-service practice the PACE model allows physicians to spend meaningful time understanding the whole personnot simply treating an isolated diagnosis.

As the Medical Director you will develop long-term relationships with participants and work alongside an interdisciplinary team to address medical functional behavioral and social needs.

Key Responsibilities

  • Conduct comprehensive in-person medical assessments and provide primary care to assigned participants.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Provide ongoing medical oversight and continuity of care.
  • Coordinate admissions and discharges involving hospitals nursing homes acute-care facilities and long-term placements.
  • Collaborate with community-based physicians specialists hospitals and post-acute providers to ensure effective transitions of care.
  • Participate actively in IDT meetings and communicate significant changes in participant condition treatment and care needs.
  • Coordinate and support delivery of appropriate 24-hour medical care.
  • Provide medical oversight for participants residing in nursing facilities including regular visits based upon clinical need.
  • Provide direction guidance and clinical support to Nurse Practitioners and other members of the interdisciplinary team.
  • Promote evidence-based approaches to chronic disease management and geriatric care.
  • Support appropriate utilization of specialists hospital services nursing facilities and other healthcare resources.
  • Maintain a strong focus on quality safety participant experience and clinical outcomes.
  • Participate in a rotating physician on-call schedule.
