This role requires a multifaceted approach encompassing all aspects of care delivery. Comprehensive geriatric assessments may be conducted in-office via telemedicine or through on-site visits at senior facilities patient homes dialysis centers and other appropriate role within your scope of practice forthe comprehensive geriatric evaluation of patients will include but not limited to obtainingvitalssigns and administering geriatric assessment toolsfollowed by sharing these results with the provider.

Essential Functions

• Conductvisits in coordination with other P3 team membersin a variety ofsettings.
• Escalate concerns youdiscoverin a patient visit tothe designated provider orappropriate teammember.
• Collaborate withthe interdisciplinaryteamto support coordinated value-based care.
• Verifies patient informationutilizing2 unique identifiers;obtain vital signs administer geriatric assessment toolsrecord medical historyand medications for review by provider and/or pharmacist.â
• Performs various procedures ( ear lavage medication administration) and specimen handling.â
• Perform administrative dutiesas necessary as a member of the team.â
• Maintains a safe secure and healthy work environment byestablishingand following standards and procedurescomplying withlegal regulations.â
• Identifiessupplies requiring ordering to theappropriate teammember.
• Keeps equipmentoperatingby following operating instructions; troubleshooting breakdowns;maintainingsupplies; performing preventive maintenance; calling for repairs.â
• Enhance practice reputation by accepting ownership foraccomplishingnew and different requests exploring opportunities to add value to job accomplishments.â
• Communicate opportunities to advance the clinical model.
• Confidently use technologyprovided.
• Ensure patient privacy and adherence to all HIPAA regulations and standards for handling PHI.
• Participate in quality improvement initiatives documentation audits and other activities that support clinical excellence and operational efficiency.
• Maintain compliance with requiredtrainingsentries into the medical record meeting attendanceand other requirements.

Qualifications

• Medical Assistant certification required
• Experience in home care or house call settings preferred
• Experience in geriatriccare preferred
• Flexible self-driven collaborative and innovative personality

Hourly pay raterange:$20 - 27.50 based on experience
