We are seeking a compassionate organized and experienced MDS Nurse/Nurse Manager to join our leadership team. This is a full-time benefit-eligible position ideal for a nurse who thrives in both clinical leadership and clinical data assessments.

Position Summary

The MDS Nurse/Nurse Manager plays a key leadership role in overseeing resident care supporting nursing staff and completing the MDS process to ensure accurate assessments care planning regulatory compliance for maximum reimbursement.

This position combines hands-on clinical leadership with MDS responsibilities and requires strong communication critical thinking and organizational skills.

What Youll Do

MDS Responsibilities

  • Coordinate and complete MDS assessments accurately and timely
  • Ensure compliance with State and Federal regulations
  • Participate in care plan meetings and interdisciplinary collaboration
  • Review documentation to support reimbursement and quality measures
  • Monitor clinical documentation for accuracy and completeness
  • Assist with audits quality improvement initiatives and survey preparedness

Nurse Manager Responsibilities

  • Provide clinical leadership and oversight for residents and staff
  • Support mentor and coach CNA and Nursing staff
  • Monitor resident conditions and communicate changes appropriately
  • Assist with assessments care planning and interventions
  • Ensure high-quality resident-centered care and compliance with facility policies
  • Help maintain staffing coverage and assist on the floor as needed during call-ins or staffing shortages
  • Foster a positive respectful team-focused culture

Qualifications

  • Current RN license in the State of Wisconsin
  • Minimumm of 2 years MDS experience required
  • Long-term care or skilled nursing experience preferred
  • Strong assessment documentation and organizational skills
  • Ability to lead mentor and communicate effectively with staff and families
  • CPR certification or willingness to obtain within 30 days of hire
  • Experience with care planning Medicare/Medicaid documentation and regulatory compliance preferred
