We are seeking a fully licensed Masters Social Worker to provide clinical and case management services for medical-psychiatric patients. The role requires strong skills in individual psychotherapy experience in inpatient care and collaboration within multidisciplinary teams.The social worker will conduct biopsychosocial assessment group therapy sessions lead individual patient-centered treatment team meetings with multi-departments involved in patient treatment and facilitate patient discharge planning and follow-up after care. This is an in-person full-time role.

Job Duties:
- Provide individual psychotherapy and case management for patients in the inpatient setting.
- Sympathetic and patient-centered care.
- Communicate with patient family guardian or other care team members involved in the patients care.
- Coordinate care across settings and connect patients to community resources.
- Establish follow-up after care appointments utilizing providers for referral requests as needed.
- Coordinate discharge planning.
- Completes all necessary paperwork thoroughly accurately and promptly as specified by the Services operating procedure also defined by insurers HIPPA MDHHS NASW standards for clinical social work MI Mental and Public Health Codes and best practice.
- Completes required documentation for record keeping and reimbursement according to the Services operating procedures.
- Maintain documentation and adhere to hospital policies and evidence-based practice standards.
- Initiates and participates in team interdisciplinary communication.
- Establishes and maintains effective working relationships with patients families co-workers and other professionals.
- Collaborate with physicians nurses psychiatrists psychologists and supportive therapy staff.
- Exercises independent judgment but utilizes appropriate consultation when indicated
Qualifications:
- Ability to demonstrate authorization to work in the U.S. for any employer.
- Masters degree from an approved school of social work (accredited by the Council of Social Work Education) is required.
- Active LCSW license preferred.
- One year of social work in a health care setting is required.
- Experience in inpatient behavioral health and substance use preferred.
- Strong individual psychotherapy skills.
- Proficiency with electronic health records and basic computer tasks
- Comfortable collaborating in multidisciplinary teams
- Communicates effectively and accurately verbally and in writing.
