Job Summary

Job Title: Master of Social Work (MSW) Travel Contract

Location: Napa CA
Contract Length: 13 Weeks
Shift: Day Shift 5x8s 8:00 AM 4:30 PM

We are seeking a compassionate and experienced Master of Social Work (MSW) professional for a travel assignment in Napa California. The ideal candidate will provide psychosocial support discharge planning care coordination and advocacy services while collaborating closely with interdisciplinary healthcare teams.

Responsibilities

  • Conduct comprehensive psychosocial assessments of patients and families

  • Develop and implement individualized care and discharge plans

  • Provide crisis intervention counseling and emotional support services

  • Assist patients with accessing community resources financial assistance programs and support services

  • Collaborate with physicians nurses case managers and healthcare professionals to coordinate patient care

  • Advocate for patients rights and promote patient-centered care

  • Facilitate discharge planning to ensure safe and effective transitions of care

  • Identify barriers to treatment and develop strategies to address patient needs

  • Maintain accurate and timely documentation in accordance with facility and regulatory standards

  • Participate in multidisciplinary team meetings and case reviews

  • Educate patients and families regarding available healthcare and community resources

  • Ensure compliance with hospital policies state regulations and ethical standards of social work practice

Requirements

  • Masters Degree in Social Work (MSW) from an accredited program required

  • Current California Social Work licensure or eligibility required

  • Previous experience in a healthcare hospital or acute care setting preferred

  • Strong knowledge of discharge planning care coordination and community resource management

  • Excellent communication interpersonal and counseling skills

  • Ability to work effectively in a multidisciplinary healthcare environment

  • Strong assessment critical thinking and problem-solving abilities

  • Experience in acute care medical social work or case management preferred

  • Familiarity with EMR systems preferred

  • Knowledge of Medicare Medicaid and healthcare regulations preferred

  • Experience working with diverse patient populations preferred
