Job Summary
Monitor and track data and metrics that demonstrate the impact of community health initiatives. Oversee local Community Health Worker program directly supervising CHWs in ED clinic and hospital. Manage and/or provide technical assistance of local grants (as needed) for community health services and patient assistance as applicable.
- Education: Bachelors degree in public or community health healthcare health education or related field required.
- Experience: 2-3 years experience in community health or related field
- Preferred Education: Master of Public Health (MPH) or Master of Healthcare Administration (MHA) preferred
- Other: Must understand English both written and spoken must be able to safely and successfully perform job-related functions with or without reasonable accommodations required by federal state or local law must be able to travel between Mercy communities.
