As our Case Management Manager you will help our team optimize the daily operations of our case management functions so they can deliver efficient compliant and high-quality patient care.

Every day you will oversee and guide our case management team. You will be expected to drive performance improvement manage staff and ensure seamless care coordination discharge planning utilization review and social work interventions in accordance with all relevant policies and regulations.

To be successful in this role you must possess strong leadership skills a deep understanding of case management principles and the ability to effectively manage and develop a team in a complex healthcare environment.

• Operational Excellence & Regulatory Compliance:Possesses strong technical and functional competence including a deep understanding of operational requirements and the ability to ensure compliance with the healthcare regulatory environment.
• Conflict Resolution & Performance Improvement:Adept at anticipating recognizing and dealing effectively with conflicts while also driving clinical performance improvement to ensure quality patient care.

• A minimum of three (3) years in a hospital-based Clinical Social Work role.
• Masters Degree in Social Work from a university or college accredited by the Council of Social Work Education.
• License Clinical Social Worker
