Duration: 16 Weeks
Shift: Night Shift 5x8 Hours 11:45 PM 7:15 AM
Weekly Pay: $1450 $1522 (Based on Experience)

We are seeking a dedicated Licensed Practical Nurse (LPN/LVN) to join a long-term care team providing compassionate nursing care to residents in a skilled nursing and memory care setting. This role focuses on delivering high-quality care to residents including those with Alzheimers disease and dementia while collaborating with an interdisciplinary healthcare team to support patient well-being and safety.

• Provide direct nursing care to residents in accordance with individualized care plans.
• Administer medications treatments and therapeutic interventions as prescribed.
• Monitor resident conditions and promptly report significant changes to the appropriate healthcare providers.
• Maintain accurate and timely patient documentation using electronic medical records.
• Assist with admissions transfers and discharge processes as needed.
• Collaborate with physicians nurses therapists and support staff to ensure coordinated resident care.
• Support residents with activities that promote comfort dignity and quality of life.
• Respond appropriately to resident needs emergencies and changes in condition.
• Adhere to facility policies nursing standards and regulatory requirements.
• Participate in quality improvement and patient safety initiatives.

• Graduate of an accredited Practical Nursing Program.

• Active New York State LPN License (license must be in good standing).
• Current BLS Certification.

• Minimum of one (1) year of Long-Term Care experience preferred.
• Candidates with recent experience in rehabilitation medical-surgical or hospital settings will also be considered.
• Experience caring for residents with Alzheimers disease and dementia preferred.

• Proof of active nursing license required at submission.

• Requested Time Off (RTO) must be submitted and approved prior to offer acceptance.
• Ability to work independently during night shifts while maintaining excellent patient care standards.
• Strong communication documentation and teamwork skills.
