Compensation: $40 $45 per Hour

Position Overview

A growing mission-driven hospice organization is seeking a compassionate and relationship-focused LVN Hospice Community Outreach Liaison to join its expanding Marin County team. This is an excellent opportunity for a hospice clinician who enjoys community engagement patient advocacy and relationship-building while continuing to make a meaningful impact on patients and families during critical moments of care.

In this role you will serve as a trusted resource for referral partners patients families and internal care teams by helping educate the community about hospice services and supporting the referral and admissions process. The ideal candidate will bring prior hospice experience strong interpersonal skills and a passion for compassionate end-of-life care.

Key Responsibilities
  • Support hospice clinical and outreach teams with patient referrals admissions coordination and community engagement initiatives
  • Build and maintain strong professional relationships with physicians hospitals skilled nursing facilities assisted living communities case managers and other referral partners
  • Educate patients families and healthcare professionals regarding hospice philosophy eligibility and available services
  • Conduct introductory meetings with patients and families and assist with hospice consent discussions
  • Participate in community outreach events networking opportunities and educational initiatives throughout the territory
  • Maintain accurate CRM documentation referral tracking and activity reporting
  • Identify referral opportunities and communicate market feedback and growth opportunities to leadership
  • Serve as a knowledgeable hospice resource for referral partners and internal team members
  • Collaborate closely with clinical and operational leadership to support exceptional patient experiences and continued organizational growth

Qualifications
Required
  • Current unrestricted LVN or LPN license
  • Minimum of 1 year of hospice experience required
  • Reliable transportation valid drivers license and current auto insurance
Preferred
  • 23 years of hospice experience preferred
  • Prior community outreach healthcare liaison or customer service experience strongly preferred
