Seneca Health Care Center is looking to temporarily add a LPN Unit Manager of Long-Term Care to its nursing leadership this role youll oversee the daily clinical operations of an assigned nursing unit ensuring high-quality resident care regulatory compliance and effective coordination among residents families providers and interdisciplinary team members. RESPONSIBILITIES:
Oversee the clinical care and daily operations of an assigned nursing unit
Supervise nursing staff nursing assistants and unit support staff while providing leadership and guidance
Monitor resident conditions review assessments and ensure care plans are updated and implemented appropriately
Coordinate communication with physicians nurse practitioners physician assistants residents and families
Review laboratory results provider orders incident reports and changes in resident condition
Participate in care plan meetings QAPI activities family meetings and interdisciplinary team discussions
Assist with admissions discharges transfers and coordination of outside appointments
Ensure unit supplies documentation and resident records are accurate and current
Support staff education orientation infection prevention efforts and quality improvement initiatives
Address resident family and staff concerns while promoting a positive care environment
Perform additional duties as assigned
REQUIREMENTS:
High School Diploma or equivalent required; Associates Degree preferred
Minimum 1 year of LPN experience in a long-term care setting required
3-5 years of long-term care experience with supervisory experience preferred
Strong knowledge of resident care planning clinical documentation and long-term care regulations
Leadership experience with the ability to supervise coach and mentor staff
Excellent communication organizational and problem-solving skills
Ability to coordinate care across multiple disciplines and manage competing priorities
