RESPONSIBILITIES:
- Provide nursing care in accordance with resident assessments care plans and facility policies
- Administer medications treatments enteral feedings and IV therapies within the LPN scope of practice
- Monitor resident conditions and promptly communicate changes to providers supervisors and responsible parties
- Accurately document care treatments observations and medication administration
- Supervise and support Nursing Assistants to ensure quality resident care
- Maintain up-to-date resident care information and communicate pertinent information during shift changes
- Follow infection control safety resident rights and confidentiality standards
- Participate in interdisciplinary care planning and quality improvement initiatives
- Assist with admissions transfers and other nursing responsibilities as assigned
REQUIREMENTS:
- High School Diploma or equivalent required; Associates Degree preferred
- Valid NYS Licensed Practical Nurse (LPN) License required
- Current CPR Certification required
- Experience in a skilled nursing facility or with the geriatric population preferred
- Strong clinical communication and organizational skills
- Knowledge of medication administration infection control and resident care practices
- Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
