Rate: $27.54- $32.77/hour (depending on experience)
Shift Differential Available for Evenings & Overnights!

Northgate Health Care Facilityis looking to add a Licensed Practical Nurse (LPN) to its nursing this role youll provide high-quality nursing care administer medications and treatments and help ensure the safety comfort and well-being of residents. RESPONSIBILITIES:

  • Provide nursing care in accordance with resident assessments care plans and facility policies
  • Administer medications treatments enteral feedings and IV therapies within the LPN scope of practice
  • Monitor resident conditions and promptly communicate changes to providers supervisors and responsible parties
  • Accurately document care treatments observations and medication administration
  • Supervise and support Nursing Assistants to ensure quality resident care
  • Maintain up-to-date resident care information and communicate pertinent information during shift changes
  • Follow infection control safety resident rights and confidentiality standards
  • Participate in interdisciplinary care planning and quality improvement initiatives
  • Assist with admissions transfers and other nursing responsibilities as assigned

REQUIREMENTS:

  • High School Diploma or equivalent required; Associates Degree preferred
  • Valid NYS Licensed Practical Nurse (LPN) License required
  • Current CPR Certification required
  • Experience in a skilled nursing facility or with the geriatric population preferred
  • Strong clinical communication and organizational skills
  • Knowledge of medication administration infection control and resident care practices
  • Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
  • Compassionate resident-centered approach to care
