Job Summary

Title: LPN Primary Care

Type: Temp to perm

Hours: 40 hours weekly Monday to Friday 8am to 430pm

Location: Plainsboro NJ

Dates: July 20 to October 24 2026

Position Summary

We are seeking an experienced and patient-focused Licensed Practical Nurse (LPN) to join our Primary Care team. This role supports providers and patients through clinical coordination patient communication and administrative support to ensure efficient day-to-day practice operations and exceptional patient care.

The ideal candidate will have strong clinical judgment excellent organizational skills and experience working within a fast-paced outpatient setting.

Responsibilities

  • Triage incoming patient calls and assess needs appropriately within scope of practice
  • Manage and respond to patient messages through the EMR in-basket system
  • Coordinate and process prior authorizations including medication requests and payer communication
  • Schedule patient appointments follow-ups and coordinate care as needed
  • Assist providers with patient flow and clinical support activities
  • Document patient interactions accurately and timely within the electronic medical record
  • Support prescription refill workflows and care coordination activities
  • Provide patient education and reinforce provider care plans
  • Occasionally assist with medical reception/front desk duties including patient check-in/check-out and administrative support
  • Maintain confidentiality and compliance with all clinical and regulatory standards

Qualifications

  • Active Licensed Practical Nurse (LPN) license in NJ
  • Minimum 2 years of LPN experience preferably in Primary Care Ambulatory or Outpatient settings
  • Experience using EPIC electronic medical record system
  • Strong communication multitasking and patient service skills
  • Experience with CoverMyMeds for prior authorization management
  • Prior experience supporting high-volume physician practices or medical groups
