As an LPN or RN on this case you will provide 1:1 nursing care to a middle-aged adult in their home. This is a great opportunity for a nurse who enjoys relationship-based care consistency and having the time to truly focus on their patients needs.

• Provide individualized skilled nursing care according to the patients plan of care

• Administer medications and treatments as ordered

• Monitor and document the patients condition and any changes

• Assist with medical equipment and other skilled nursing needs as appropriate

• Communicate with the patient family and Castor clinical team

• Complete accurate electronic documentation

• Provide compassionate respectful care while promoting the patients comfort safety and independence
