Job description:
LPN LTC Job Responsibilities
The Licensed Practical Nurse (LPN) in a Long-Term Care setting provides direct nursing care under the supervision of an RN or physician ensuring residents receive safe compassionate and high-quality care.
- Provide direct bedside nursing care to LTC residents under RN supervision
- Administer medications and treatments as prescribed
- Monitor and document resident vital signs symptoms and clinical status
- Report any changes in condition promptly to the supervising RN or charge nurse
- Assist with and reinforce care plans developed by the RN team
- Provide wound care and basic nursing interventions as directed
- Support residents with activities of daily living (ADLs) when needed
- Ensure accurate and timely documentation in medical records/EMR
- Observe and follow infection control and safety protocols
- Assist with admission transfer and discharge of residents
- Collaborate with CNAs and other care team members to ensure continuity of care
- Promote resident comfort dignity and quality of life
- Respond appropriately to urgent or emergency situations within scope of practice
- Maintain compliance with facility policies and state nursing regulations.
Requirements
- Active LPN License
- Experience in Long-Term Care or Skilled Nursing preferred
- Strong clinical judgment and patient care skills
- Ability to work scheduled weekend shifts (FriSun)
