Position Overview:
Navitas Healthcare LLC is seeking Licensed Practical Nursefor an exciting Travel or Local job in Lewiston ME.
Responsibilities
Provide direct nursing care to residents in Long-Term Care (LTC) Skilled Nursing (SNF) Dementia Care and Rehabilitation units.
Administer medications treatments and nursing interventions as prescribed.
Monitor resident conditions and promptly report changes to the healthcare team.
Assist with resident assessments care planning and implementation of individualized care plans.
Provide specialized care to residents with Alzheimers disease and dementia utilizing appropriate communication and behavioral management techniques.
Deliver nursing care to rehabilitation patients recovering from surgeries fractures strokes and other acute illnesses.
Perform wound care IV therapy and geriatric assessments as required.
Maintain accurate and timely documentation within the facilitys electronic medical record system.
Collaborate effectively with physicians RNs CNAs therapists and other interdisciplinary team members.
Promote resident safety dignity comfort and quality of life.
Float between units as needed to support patient care and staffing requirements.
Participate in resident and family education regarding care plans and treatment goals.
Required Qualifications
Education:
Associate Degree in Nursing (ADN) or Graduate of a Maine State Board Approved Practical Nursing Program.
Licensure / Certification:
Active Maine LPN License or Compact LPN License Required.
Basic Life Support (BLS/BCLS) Certification Required.
Experience:
Minimum 13 years of previous LPN or RN experience preferred.
Experience in Long-Term Care (LTC) Skilled Nursing Facility (SNF) Rehabilitation Geriatrics or Dementia Care preferred.
Experience with EPIC EMR preferred.
Preferred Qualifications
Strong communication and interpersonal skills.
Ability to work effectively in a fast-paced healthcare environment.
Experience caring for geriatric populations and residents with cognitive impairments.
Ability to work independently while collaborating with interdisciplinary teams.
