Job Title: LPN - Long Term Care (LTC)
Location: Chapman KS
Type: Contract 8 Weeks
Shift & Schedule: Variable Nights 12-Hour Shifts

Position Overview:
Navitas Healthcare LLC is seeking LPN - Long Term Care (LTC) for an exciting contract opportunity in Chapman KS.

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans
• Administer medications and treatments as prescribed
• Monitor assess and document resident conditions and responses to care
• Assist with admissions transfers and discharge processes
• Perform routine clinical procedures and nursing interventions
• Communicate changes in resident condition to physicians and nursing leadership
• Maintain accurate documentation and resident records
• Collaborate with interdisciplinary healthcare teams to support quality resident care
• Ensure compliance with infection control safety and facility policies

Requirements

• Active LPN license required
• Long Term Care (LTC) experience required
• Strong medication administration and documentation skills
• Excellent communication and patient care skills
• Ability to work independently in a fast-paced environment
• Strong organizational and time management skills
• Ability to work variable night shifts and weekends as needed
