LPN – Long Term Care (LTC) LPNVC

Job Title: LPN Long Term Care (LTC)

Location: New Lisbon WI

Contract Duration: 13 Weeks

Shift:
PM Shift: 2:00 PM 10:00 PM
Every Other Weekend Required

Start Date: 06/01/2026

Responsibilities:
Administer medications following the 5 Rights of Medication Administration
Monitor and report changes in resident condition to Charge Nurse and physicians
Communicate resident updates with family members as needed
Assist CNAs with call lights and resident care support
Report abuse neglect or misappropriation according to facility policy
Maintain accurate patient documentation
Support a safe and compassionate care environment
Perform additional nursing duties as assigned

Requirements:
Active Wisconsin LPN License required
Current BLS/CPR Certification required
Long Term Care experience preferred
Minimum 1 year of related nursing experience preferred
Strong attention to detail and communication skills required
Ability to work rotating weekends and holiday coverage as needed
