Position Overview

The Licensed Practical Nurse (LPN) Long-Term Care (LTC) provides direct nursing care to residents while supporting the delivery of high-quality patient-centered care in a long-term care setting. This role collaborates with Registered Nurses physicians nursing assistants and interdisciplinary teams to ensure residents receive safe compassionate and effective care that promotes optimal health outcomes and quality of life.

Responsibilities

• Provide direct patient care in accordance with individualized care plans and facility policies.
• Administer medications and treatments as prescribed and within scope of practice.
• Monitor resident conditions and promptly report changes to the healthcare team.
• Assist with resident assessments care planning admissions transfers and discharges.
• Maintain accurate and timely documentation of resident care and clinical observations.
• Collaborate with RNs physicians therapists and other healthcare professionals to support resident outcomes.
• Provide patient and family education regarding treatments medications and care plans.
• Ensure compliance with facility policies infection control practices and regulatory requirements.
• Assist in supervising and supporting nursing assistants and ancillary staff.
• Promote a safe respectful and resident-focused care environment.

Required Qualifications

• Graduate of an accredited Practical Nursing (LPN) Program.
• Nursing Diploma or Certificate from an approved institution.
• Active Licensed Practical Nurse (LPN) License Required.
• BLS Certification Required.
• COVID-19 Vaccination Documentation Required.
• Long-Term Care (LTC) experience preferred.
• Strong commitment to providing compassionate resident care.
• Excellent communication documentation and patient care skills.

Preferred Qualifications

• Previous Long-Term Care (LTC) experience.
• Experience working in skilled nursing or rehabilitation settings.
• New graduates and candidates with less than one year of experience may be considered.
