Pay Rate: $1500 $2100/week (Depending on Experience)

Position Overview

Navitas Healthcare LLC is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join a long-term care team in Falmouth Massachusetts. The ideal candidate will provide quality nursing care support resident well-being and collaborate effectively with physicians nursing assistants and interdisciplinary healthcare teams.

Key Responsibilities
  • Direct the day-to-day functions of nursing assistants in accordance with facility policies and regulatory guidelines.
  • Coordinate resident care activities and support quality patient care initiatives.
  • Encourage attending physicians to review treatment plans progress notes and physician orders.
  • Receive and accurately document physician telephone orders.
  • Meet with assigned nursing staff and support personnel to plan shift activities and resident care services.
  • Complete and maintain required admission transfer and discharge documentation.
  • Monitor resident conditions and report changes to appropriate healthcare providers.
  • Ensure compliance with facility policies state regulations and patient safety standards.
  • Maintain accurate and timely nursing documentation.
Required Qualifications
Licensure & Certifications
  • Current active and unrestricted Massachusetts LPN License.
  • BLS / CPR Certification.
  • AED Certification.
Education
  • Graduate of an approved Licensed Practical Nursing (LPN) Program.
  • Nursing Degree from an accredited college or university preferred.
Experience
  • Minimum one (1) year of LPN experience required.
