Acclaim Home Health is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our home health care team in Worcester MA. This role involves providing high-quality nursing care to patients in the comfort of their own homes ensuring their health and well-being through skilled and personalized care.

• Provide skilled nursing services to patients in their homes including medication administration wound care and patient education.
• Monitor and document patient conditions and progress accurately.
• Collaborate with healthcare professionals to develop and implement personalized care plans.
• Ensure compliance with all regulatory and company standards.
• Communicate effectively with patients and their families to support health goals and encourage adherence to treatment plans.

• Current LPN license in the state of Massachusetts.
• Previous experience in home health or a related nursing field preferred.
• Strong interpersonal and communication skills.
• Ability to work independently and manage time effectively.
• Valid drivers license and reliable transportation.
