• Provide high-quality skilled nursing care to patients in their homes

• Administer medications and treatments as ordered

• Perform assessments monitor changes and communicate updates to the care team

• Educate patients and families on disease processes medications and self-care

• Collaborate with RNs therapists physicians and caregivers to support the care plan

• Document accurately and timely according to state federal and agency standards

• Ensure all patient care meets regulatory and clinical guidelines

• Current LPN license in good standing (Oklahoma)

• Home health experience preferred (we also welcome strong clinical LPNs ready to transition into home health)

• Excellent communication and organizational skills

• Ability to work independently with confidence and sound judgment

• Passion for providing compassionate patient-centered care

• Reliable transportation valid drivers license and current auto insurance
