Job Overview

We are seeking a Licensed Practical Nurse (LPN) for a local contract assignment in a skilled nursing and long-term care setting. The LPN will provide direct patient care administer medications monitor resident conditions and support the nursing team in delivering safe and effective care.

This role follows a rotating schedule and is best suited for nurses comfortable working both weekday and weekend rotations.

Key Responsibilities

Administer medications and prescribed treatments
Monitor assess and document patient conditions and changes
Provide direct nursing care according to care plans
Assist with admissions discharges and resident assessments
Communicate updates to supervising RN and care team
Maintain accurate and timely clinical documentation
Support infection control and safety protocols
Collaborate with interdisciplinary healthcare staff

End Date: 07/21/2026

Shift: Day/Night Rotation
Shift Time: 07:00 19:30

Guaranteed Hours: 36 Hours per Week

Requirements

Active Licensed Practical Nurse (LPN) license required
Long-term care experience preferred
Ability to work rotating schedules (including weekends)
Strong clinical and communication skills
Must comply with onboarding requirements
Must bring state or federally issued identification and wear a photo badge at all times
COVID-19 vaccination or approved exemption may be required per policy
