Your Role & Responsibilities:
- Companionship & Connection: Engage clients in meaningful conversation puzzles walks and activities to keep them socially active and mentally sharp.
- Dignified Personal Care: Provide hands-on support with Activities of Daily Living (ADLs) including bathing grooming dressing skin care and personal hygiene.
- Safe Mobility & Transfers: Assist clients with ambulation transfers and positioning using proper body mechanics and assistive devices.
- Meal Prep: Assist with meal planning and nutritional cooking according to dietary restrictions; monitor and encourage hydration.
- Medication Reminders: Provide timely reminders for clients to take prescribed medications and support routine adherence.
- Light Housekeeping: Maintain a safe clean and hygienic environment by handling laundry and light cleaning.
- Companionship & Transport: Engage clients in meaningful social interaction and accompany or drive them to local medical appointments and essential outings.
- Clinical Observation: Closely observe document and report any changes in the clients physical or mental status to the nursing supervisor.
- Active New Hampshire LNA (Licensed Nursing Assistant) certificate in good standing (Required).
- 1 years of experience as an LNA in home health a facility or a hospital setting preferred.
- Current certification in CPR and First Aid (Preferred).
- Strong communication skills and a reliable work ethic.
