POSITION SUMMARY The Integrated Health Care (Outpatient) Clinician provides clinical and case management services for children and adults across the lifespan in an integrated health care setting that provides primary care behavioral health and dental services. The Integrated Health Care Clinician will foster positive working relationships with co-workers clients and external key stakeholders.$10000 sign on bonus available! Half paid at start the other half paid at 1 year of service! Federally Qualified Health Center (FQHC) in Bristol CT. Eligible for up to $250000 in loan repayment through National Health Service Corp (NHSC).SALARY:$60000.00 for Associate Licensure (LMSW LPCA LMFTA)$73500.00 for Full Licensure (LCSW LPC LMFT). Demonstrates knowledge of psychopathology and primary modalities of psychotherapy including individual group and family. Correctly triages clinical emergencies including suicide and danger to others. Uses best practices and evidenced based practice models for psychotherapy where appropriate. Makes effective use of topic oriented focused time-limited group or refers to group when required. Demonstrates knowledge of the use of psychotropic medications. Completes clinical documentation in timely manner. Engages in facilitating group (outpatient or intensive outpatient) as well as seeing patients across the lifespan when knowledge of internal and external levels of care referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely in a positive effective manner with client family and all internal and external providers. Attends any required meetings and multidisciplinary treatment team planning direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes and adheres to practice standards and to data collection and reporting all client related information in compliance with agency and is prepared for scheduled supervision. Seeks additional supervision or consultation as needed and follows through with supervisory and collaborates with community providers to provide positive system development for clients and families to promote Wheeler Clinics reputation as a quality service provider. Provides culturally competent care and maintains sensitivity to and respect for clients cultural traditions values and child abuse/neglect as mandated adheres to confidentiality guidelines and ensures that the program meets all necessary licensing regulatory and accreditation standards as related to the program staff records facility and care of to develop knowledge and understanding about the history traditions values family systems and artistic expression of groups served as well as uses appropriate methodological approaches skills and techniques that reflect an understanding of culture.
