Job Summary

Position: Licensed Vocational Nurse
Location: Los Angeles CA 90069 & West Hollywood CA 90069
Overview:
The Licensed Vocational Nurse (LVN) is responsible for providing individualized patient care promoting and restoring patient health through nursing interventions collaborating with healthcare providers and multidisciplinary teams and offering physical and emotional support to patients.
This role requires strong clinical skills excellent patient communication and the ability to work effectively in a fast-paced outpatient healthcare environment.
Key Responsibilities:
Clinical Care
  • Perform nursing duties in accordance with established protocols physician orders and applicable nursing regulations.
  • Conduct patient assessments and provide treatments within scope of practice.
  • Administer medications collect blood samples and initiate IV access when appropriately certified.
  • Review laboratory results and report abnormal findings to providers.
  • Perform medication reconciliation and assist with care coordination.
  • Provide patient education regarding medical conditions medications treatment plans and self-care.
Patient Support & Care Coordination
  • Triage patient phone calls patient portal messages and voicemail inquiries.
  • Assist with patient follow-up and outreach activities to ensure continuity of care and appointment compliance.
  • Coordinate with multidisciplinary teams to address patient needs and resolve care-related concerns.
  • Provide compassionate non-judgmental and culturally sensitive care to diverse patient populations.
Administrative & Operational Support
  • Process required public health reporting and clinical documentation.
  • Assist with prior authorizations and other clinical administrative tasks.
  • Document patient care accurately within medical records and clinical systems.
  • Maintain continuity of care through effective communication with healthcare teams.
  • Support inventory management of nursing supplies and equipment.
  • Assist in ensuring efficient clinic operations and patient flow.
  • Provide oversight and guidance to Medical Assistants as directed by nursing leadership.
Compliance & Quality Assurance
  • Adhere to infection-control standards medication administration protocols and safety procedures.
  • Maintain compliance with HIPAA OSHA and all applicable healthcare regulations.
  • Protect patient confidentiality and privacy.
  • Participate in staff meetings trainings case conferences and continuing education activities.
  • Maintain professional licensure and required certifications.
Qualifications:
Required
  • Active Licensed Vocational Nurse (LVN) license in the State of California.
  • Current Basic Life Support (BLS) certification.
  • Minimum of two (2) years of direct patient care experience as an LVN.
  • Strong knowledge of clinical procedures medical terminology and patient care practices.
  • Excellent interpersonal and communication skills.
  • Strong attention to detail and organizational skills.
Preferred
  • Experience managing patients with HIV/AIDS Hepatitis C chronic illnesses or infectious diseases.
  • Knowledge of state and federal reporting requirements for communicable diseases.
  • Additional clinical certifications are considered a plus.
