Licensed Social Worker (LSW)

We are seeking a compassionate and dedicated Licensed Social Worker (LSW) to provide psychosocial support counseling and care coordination services to patients and their families. The ideal candidate will assess patients social emotional and environmental needs and collaborate with interdisciplinary healthcare teams to develop and implement effective care plans.

• Conduct comprehensive psychosocial assessments of patients and families.
• Develop implement and monitor individualized care and discharge plans.
• Provide counseling crisis intervention and emotional support services.
• Coordinate community resources and referrals to support patient needs.
• Advocate for patients and families to ensure access to appropriate services.
• Collaborate with physicians nurses case managers and other healthcare professionals.
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
• Assist patients with transition planning including discharge and post-discharge support.
• Educate patients and families regarding available resources benefits and support programs.
• Participate in interdisciplinary team meetings and case reviews.

• Bachelors or Masters Degree in Social Work (BSW or MSW) from an accredited institution.
• Current and active Licensed Social Worker (LSW) license in the state of practice.
• Minimum of 1-2 years of social work experience preferred.
• Experience in healthcare hospital long-term care rehabilitation behavioral health or community settings preferred.
• Strong assessment counseling and case management skills.
• Excellent communication interpersonal and organizational abilities.
• Knowledge of healthcare regulations community resources and discharge planning processes.
